My logo will sometimes disappear on some pages, and my site name will just show up there. How can I stop this from happening?

Just installed it on my site. My logo shows on all my pages. On which pages doesn't it show on your site? And what is the url?

Edit: what did you put in the field for your your site banner? Is it "images/file.ext"?
