Noobie question - website membership application

I am currently involved in the administration of a website for a local cycling club and I am still close to the bottom of the learning curve when it comes to adding features. The website is currently running PHP Fusion V7.00

The website is set up such that only club members are given login details to the website. Currenly, in order to become a member you have to download our application form and send to the club secretary. I am wondering if there is a slicker way to do this with a custom page which would have the necessary fields (Name, address phone number etc.) which are filled in then add this data to a database of some sort or emails this to the info@ email address for a club admin to process.

Any pointers (expressed in simple terms) would be greatly apreciated.

I totally understand that. I'm new to PHP, SQL, and PHPFusion... I'm not genius, but I got my custom page form submitting data to a table. Send me a PM if ya want a hand.
