set_meta functions not working while add_to functions do

Not sure if this is in the correct area, but I wasn't sure where to post this;
For some add reason the set_meta functions don't work. I'm attempting to add set_meta("description",""); for SEO and it doesn't update the description.
The add_to_title function works just fine and I notice that they are both handled by the handle_output function.
Anyone know what could be up with this?
