Anyone ever see anything like this before?

no but I think I did find the answer "will Happen when Your Sql Host puts an Hourly Restriction Limit on your Database Activity."

now just need the solution. most likely need to work it out with the host. It happened when I tried to run the installer for the "users_country_panel"

Simple solution: I went into my host's control panel (vDeck) and edited the database user's permissions (don't need to change anything, just hit edit and then okay) all is sudden everything worked fine with the site and the permissions / questions issue simply dissapeared.......

Maybe it triggers a reset that you cant notice in your panel.
Nothing happens without a reason tbh & nothing simply disappaers ;)
